School Health Immunization Requirements
For attendance in ALL grades, children are required to have the following:

4 doses of tetanus, diphtheria and acellular pertussis*
- 4 doses of polio**

2 doses of measles, mumps and rubella (MMR)
- 3 doses of hepatitis B

2 doses of varicella (chicken pox) vaccine or evidence of immunity
Children entering 7th grade will need the following:

1 dose of tetanus, diphtheria, acellular pertussis (Tdap) (if 5 years have elapsed since last tetanus immunization)

1 dose of meningococcal conjugate vaccine (MCV)
- 2nd dose of meningococcal conjugate vaccine (MCV)*
* If one dose was given at age 16 or after, that shall count as the 12th grade dose.
These requirements allow for exemptions due to medical reasons or religious/ethical beliefs. If your child is exempt from immunizations, he/she may be excluded from school during a vaccine-preventable disease outbreak.
