Attention Parents and Guardians
 
 

 School Health Immunization Requirements  

For attendance in ALL grades, children are required to have the following:

  • 4 doses of tetanus, diphtheria and acellular pertussis* 
  • 4 doses of polio** 
  • 2 doses of measles, mumps and rubella (MMR)
  • 3 doses of hepatitis B
  • 2 doses of varicella (chicken pox) vaccine or evidence of immunity
*1 dose on or after the 4th birthday
**4th dose on or after the 4th birthday and at least 6 months after the previous dose was given, unless dose 3 was given after age of 4
 

Children entering 7th grade will need the following:

  • 1 dose of tetanus, diphtheria, acellular pertussis (Tdap) (if 5 years have elapsed since last tetanus immunization)
  • 1 dose of meningococcal conjugate vaccine (MCV)
 
 ​​  Children entering 12th grade will need the following: 
  • 2nd dose of meningococcal conjugate vaccine (MCV)*

​* If one dose was given at age 16 or after, that shall count as the 12th grade dose.

 

These requirements allow for exemptions due to medical reasons or religious/ethical beliefs.  If your child is exempt from immunizations, he/she may be excluded from school during a vaccine-preventable disease outbreak.
