Medication Policy

 

All medications shall be administered on a voluntary basis by the school nurse or a designee. 

 

Only medication that MUST be given during school hours will be administered.  Medication prescribed once, twice, or three times a day can be given at home unless specifically ordered otherwise by the physician.

 

All medications, including over-the-counter medicine, to be administered by the school nurse, designee, or student shall be accompanied by:

 

A.  The written request of the parent or guardian giving permission for such administration and relieving the Board and its employees of liability for such administration.


B.  A written order of a prescribing physician which shall include the purpose of the medication, the dosage, the time(s) of administration, length of period for which medication is prescribed, possible side effects, and any special circumstances.


C.  Medication will be in a clearly labeled original pharmacy container*.

(*Please do not send medication in to the school with your child. Contact the the school nurse for further instructions.)


D.  The parent of guardian of a student assumes responsibility for informing the school on any change in medication requests.  A change in medication will necessitate new written authorization from the parent or guardian and physician.
